Job summary

Jobtailor in New York seeks a Master of Science in Nursing nurse practitioner to lead lung cancer screening consultations and LDCT eligibility assessments within a multidisciplinary team.

You will provide CMS-compliant documentation, smoking cessation counseling, and coordinate referrals while contributing to quality improvement initiatives and program performance monitoring. NY NP license and FNP board certification are required.

Qualifications

• Master of Science in Nursing required.
• NY Nurse Practitioner license or willingness to obtain.
• FNP board certification through ANCC or AANP.
• Therapeutic experience working with Mental Health patients is a plus.
• Experience in pulmonology, oncology, primary care, preventive medicine, or population health preferred.
• Experience working with multidisciplinary teams and quality improvement initiatives.

Responsibilities

• Conduct comprehensive lung cancer screening consultations and assessments.
• Perform and document CMS-compliant shared decision-making visits for eligible patients.
• Evaluate eligibility for low-dose CT screening based on current guidelines.
• Order and interpret screening-related diagnostic studies within scope of practice.
• Communicate screening results and develop follow-up care plans.
• Manage patients with abnormal findings and coordinate referrals to specialty care.
• Provide smoking cessation counseling and connect patients with resources.
• Collaborate with primary care practices to improve screening rates and reduce disparities.
• Participate in quality improvement initiatives and program performance monitoring.
• Maintain accurate clinical documentation and ensure regulatory compliance.
• Support patient navigation to improve follow-up adherence.
• Engage in community outreach and education to increase awareness.
• Work with PCPs, pulmonologists, radiologists, thoracic surgeons, oncology specialists, and care management teams through screening and diagnostics.
