Job summary

Regency Integrated Health Services is seeking a Nurse Practitioner to deliver provider-level care coordination at Windsor Nursing and Rehabilitation Center of Bastrop, Bastrop, TX. The NP will work within the Texas ISNP model, ensuring ethical practice and comprehensive patient care during standard hours and on-call rotations.

Qualified candidates hold a BSN and MSN with current Board Certification, plus Texas RN/APN licensure and prescriptive authority.

Qualifications

• BSN and MSN with current Board Certification in Nursing.
• RN and APN licensure in Texas with prescriptive authority.
• Active Texas APRN and ongoing education requirements for licensure.

Responsibilities

• Maintain professionalism reflecting organization missions and facility policies.
• Attend daily meetings and coordinate care with facility staff.
• Provide provider-level care and disease management for ISNP Plan Members.
• Participate in on-call rotation for after-hours coverage.
• Document patient care timely in designated EMR systems.
• Review medications and orders; respond to abnormalities per BON scope.
