Job summary

Qualifications

Responsibilities

  • Assess patient status by history and exam, including risk factors and psychosocial factors.
  • Orders/performs diagnostic tests and coordinates with physicians for evaluation.
  • Develops and implements treatment plans; provides patient education on medications and care.
  • Participates in peer reviews, staff education, and quality assurance activities.
  • Advocates for patient rights; maintains HIPAA compliance and professional communication.
  • Communicates case management information to professionals and community agencies.
  • Updates medical records in EMR with patient data, tests and outcomes.
