Job summary

Responsibilities

• Independently manages a patient panel with acute and chronic conditions focusing on health promotion and disease prevention.
• Develops and implements individualized plans to achieve optimal population health and disease management.
• Demonstrates leadership and collaborates with the interdisciplinary team.
• Orders and interprets labs, tests, and diagnostic studies.
• Prescribes pharmacologic and non-pharmacologic interventions.
• Initiates referrals to specialty services and educates patients and families.
