Optum is seeking a Nurse Practitioner to join our HouseCalls team as a Statewide Traveler in New Jersey. Optum is a clinician-led care organization creating a seamless health journey for patients across the care continuum.

As a member of the broader Home and Community Care team, you’ll provide annual clinical assessments to patients in their homes. This preventive care helps identify and reduce health risks and coordinate follow-up care to improve health and well-being.

This role offers work-life balance with flexible scheduling over a 7-day week, no on-call, weekends, or holidays required. Travel across New Jersey is approximately 85%, with all expenses covered. Compensation includes a base salary plus incentives.

Primary Responsibilities
  • Conduct in-home assessments including evaluation of medical history, review of systems, medication reconciliation, vital signs, and physical exams.
  • Perform evidence-based screenings, including point-of-care testing as appropriate.
  • Identify diagnoses for care management and active medical management.
  • Formulate a list of current and past medical conditions based on assessments.
  • Communicate findings to inform the patient's PCP and identify care gaps.
  • Identify urgent/emergent situations and intervene appropriately.
  • Educate members on disease processes, medications, and compliance.

Required Qualifications
  • Active NP license or ability to obtain by start date, free of disciplinary actions.
  • Active ANCC or AANP certification in Family, Geriatrics, Adult, or Adult-Gerontology Primary Care, or ability to obtain certification/license in the state.
  • Physical ability to lift 30 pounds, navigate stairs, and perform physical assessments.
  • Reliable transportation to travel up to 85% within the state.
Preferred Qualifications
  • 1+ years of clinical experience at the highest level of education.
  • Experience in Gerontology, Cardiology, Internal Medicine, or Endocrinology.
  • Home health or home visit experience.
  • Additional licensure in other geographic areas may be requested.
  • Note: Employees must remain in role for at least 12 months; leaving earlier affects student loan repayment benefits.

The salary range is $89,800 to $176,700 annually, based on full-time employment and various factors. Benefits include comprehensive packages, incentives, stock purchase, and 401k.
