Job description

Optum is seeking a Nurse Practitioner, HouseCalls - RWT - West Region (AZ, CA, HI, ID, MT, NV, OR, UT, WA, WY) in Seattle Hill, WA. This local opportunity with ID 3181139885 is live since 2025-05-04 23:41:13.

About the Role

Optum is a clinician-led care organization creating a seamless health journey for patients across the care continuum. As part of the Home and Community Care team, you'll provide annual clinical assessments in patients' homes, helping to identify health risks and coordinate follow-up care. Our holistic approach addresses physical, mental, and social needs, ensuring access and navigation of care anytime, anywhere.

Responsibilities
  1. Conduct in-home assessments including medical history, review of systems, medication reconciliation, vital signs, and physical exams.
  2. Perform evidence-based screenings and point of care testing as appropriate.
  3. Identify diagnoses for care management and active medical management.
  4. Communicate findings to inform the patient's PCP and identify care gaps.
  5. Recognize urgent/emergent situations and intervene appropriately.
  6. Educate patients on disease processes, medications, and compliance.
Qualifications
  • Active NP license or ability to obtain by start date, free of disciplinary actions.
  • Active ANCC or AANP certification in relevant specialties or ability to obtain certification/license.
  • Physical ability to perform assessments, including lifting and navigating various dwelling conditions.
  • Reliable transportation to travel up to 85% within licensed states.
Preferred Skills
  • Additional licensure in other geographic areas.
  • At least 1 year of clinical experience in relevant settings.
  • Experience in Gerontology, Cardiology, Internal Medicine, or Endocrinology.
  • Home health or home visit experience.
Compensation & Benefits

The salary range is $89,800 to $176,700 annually, based on full-time employment, local labor markets, education, experience, and certifications. Benefits include a comprehensive package, incentive programs, stock purchase, and 401(k). Employees must be in active regular status, with a minimum of 12 months in the role, and 36 months in an advanced practice role for full student loan repayment benefits.
