Nurse Practitioner HouseCalls Mid-West Regional Traveler - IA KS MN MO ND NE SD WI

Join our team as a Midwest Regional Traveler Nurse Practitioner with Optum, covering KS, MO, IA, MN, NE, ND, SD, and WI. This role involves providing in-home preventive care assessments, helping identify health risks, and coordinating follow-up care to improve patient well-being.

Enjoy flexible scheduling, no on-call, weekends, or holidays, with approximately 85% travel across multiple states. All travel expenses are covered, and the compensation includes a competitive salary plus incentives.

Responsibilities include:

• Conducting comprehensive in-home assessments
• Performing evidence-based screenings
• Identifying diagnoses and managing care
• Communicating findings to primary care providers
• Recognizing urgent situations and intervening
• Educating patients on health topics

Qualifications:

• Active NP license or ability to obtain one by start date
• National certification in Family, Geriatrics, Adult, or related fields
• Physical ability to perform assessments and navigate various home environments
• Reliable transportation for travel within licensed states

Preferred:

• Additional licensure in other states
• One or more years of clinical experience
• Experience in geriatrics, cardiology, internal medicine, or endocrinology
• Home health or home visit experience
