Optum is seeking a Mid-Atlantic RWT to join our HouseCalls team in DC, DE, MD, PA, VA. Optum is a clinician-led care organization, creating a seamless health journey for patients across the care continuum.

As a member of the broader Home and Community Care team, you’ll provide annual clinical assessments to patients in their homes. This preventive care helps identify and reduce health risks, and coordinate follow-up care to improve health and well-being.

This role offers work-life balance with flexible scheduling over a 7-day week, no on-call, weekends, or holidays required.

It involves multi-state travel across the Mid-Atlantic, approximately 85% of the time. All travel expenses are covered, with a competitive salary plus incentives.

Primary Responsibilities
  1. Conduct in-home assessments: medical history, review of systems, medication reconciliation, vital signs, physical exam.
  2. Perform evidence-based screenings, including point of care testing as appropriate.
  3. Identify diagnoses for care management and treatment.
  4. Develop a list of current and past medical conditions using clinical judgment and assessment findings.
  5. Communicate assessment findings to inform the patient's PCP and identify care gaps.
  6. Recognize urgent/emergent situations and intervene appropriately.
  7. Educate members on disease processes, medications, and adherence.

Required Qualifications
  • Active NP license or ability to obtain by start date, free of disciplinary actions.
  • NPs: Autonomous practice approval from licensing board if applicable; new hires must apply within 1 month if not already authorized.
  • Active ANCC or AANP certification in relevant specialties or ability to obtain it by start date.
  • Physical ability to perform job duties, including lifting, navigating stairs, and physical assessments.
  • Reliable transportation for travel up to 85% within licensed states.
Preferred Qualifications
  • Additional licensure in other states.
  • 1+ years of clinical experience in relevant settings.
  • Experience in Gerontology, Cardiology, Internal Medicine, or Endocrinology.
  • Home health or home visit experience.
  • Note: Employees must stay in role for at least 12 months; full benefits require a 36-month commitment.

The salary range is $89,800 - $176,700 annually, based on full-time employment and various factors. Benefits include comprehensive packages, incentives, stock purchase, and 401k. We are an equal opportunity employer committed to diversity. All candidates must pass a drug test before employment.
