Optum is seeking a Nurse Practitioner to join our House Calls team in Brevard and Indian River Counties, FL. Optum is a clinician-led care organization creating a seamless health journey for patients across the care continuum.

As a member of the broader Home and Community Care team, you’ll provide annual clinical assessments to patients in their homes. This preventive care helps identify and reduce health risks and coordinate follow-up care to improve health and well-being.

This role offers a good work-life balance with flexible scheduling over a 7-day week. No on-call, weekends, or holidays required.

Primary Responsibilities:

  • Conduct in-home assessments including medical history, review of systems, medication reconciliation, vital signs, and physical exam
  • Perform evidence-based screenings, including point of care testing as appropriate
  • Identify diagnoses for care management and treatment
  • Document findings to inform members’ PCP of care gaps
  • Identify urgent situations and intervene as needed
  • Educate members on disease, medication, and compliance topics

Required Qualifications:

  • Active NP license or ability to obtain by start date, free of disciplinary actions
  • Autonomous practice approval from licensing board if applicable
  • Active ANCC or AANP certification in Family, Geriatrics, Adult, or related fields, or ability to obtain it
  • Physical ability to perform assessments, including moving equipment and navigating various home environments
  • Driver’s license and reliable transportation for home visits

Preferred Qualifications:

  • Additional licensure in other areas if needed
  • 1+ years clinical experience in relevant settings
  • Experience in geriatrics, cardiology, internal medicine, endocrinology
  • Home health or home visit experience
  • Willingness to travel statewide up to 5%, with additional compensation possible
  • Fluency in Spanish, Cantonese, Korean, Vietnamese, Polish, or other languages

In Florida, this position requires an AHCA Level II background check (fingerprinting) for those with direct patient contact or access to confidential data, renewed every five years.

The salary range is $104,500 to $156,000 annually, based on experience and performance. Benefits include comprehensive packages, incentive programs, stock options, and 401k. Pay varies by market and qualifications.
