About

$40,000 Student Loan Repayment or $25,000 Sign-on Bonus for individuals who have not previously participated in this program.

This role requires travel across the entire state of Colorado, approximately 85% of the time. All travel expenses are covered, and there is a base salary plus multiple incentives.

Optum is seeking a Nurse Practitioner Statewide Traveler to join our HouseCalls team in Colorado. Optum is a clinician-led care organization creating a seamless health journey for patients across the care continuum.

As a member of the broader Home and Community Care team, you’ll provide annual clinical assessments to patients in their homes. This preventive care helps identify and reduce health risks and coordinate follow-up care to improve health and well-being.

In this role, you will enjoy work-life balance with flexible scheduling over a 7-day week. No on-call, weekends, or holidays required.

Primary Responsibilities

  1. Conduct in-home assessments including evaluation of medical history, review of systems, medication reconciliation, vital signs, and physical exam.
  2. Perform evidence-based practice screenings including point of care testing as appropriate.
  3. Identify diagnoses for care management and treatment.
  4. Formulate a list of current and past medical conditions based on assessments.
  5. Communicate findings to inform the patient’s PCP of care gaps.
  6. Identify urgent/emergent situations and intervene appropriately.
  7. Educate members on disease processes, medications, and compliance.

Required Qualifications

  • Active NP license or ability to obtain by start date, free of disciplinary actions.
  • Active ANCC or AANP certification in Family, Geriatrics, Adult, or Adult-Gerontology primary or acute care, or ability to obtain certification/license in Colorado.
  • Physical ability to perform assessments, lift 30 pounds, navigate stairs, and sit/stand/kneel as needed.
  • Reliable transportation to travel up to 85% within Colorado.

Preferred Qualifications

  • 1+ years of clinical experience in relevant settings.
  • Experience in Gerontology, Cardiology, Internal Medicine, or Endocrinology.
  • Home health or home visit experience.
  • Additional licensure in other areas may be requested.
  • Note: Employees must stay in role at least 12 months to retain student loan benefits, and 36 months for full benefit.

The salary range is $89,800 to $176,700 annually, based on full-time employment and various factors. Benefits include comprehensive packages, incentives, stock purchase options, and 401k contributions.
