Job Opportunity: Nurse Practitioner Statewide Traveler in Colorado

$40,000 Student Loan Repayment or $25,000 Sign-on Bonus available for eligible individuals. This role involves travel across Colorado, approximately 85% of the time, with all travel expenses covered. It offers a base salary plus multiple incentives.

This role offers flexible scheduling, no on-call, weekends, or holidays, supporting work-life balance.

Primary Responsibilities:
  1. Conduct in-home assessments, including medical history review, vital signs, and physical exams.
  2. Perform evidence-based screenings and point of care testing as appropriate.
  3. Identify diagnoses for care management and treatment planning.
  4. Document current and past medical conditions based on assessments.
  5. Communicate findings to inform the patient's PCP and identify care gaps.
  6. Recognize urgent situations and intervene as needed.
  7. Educate patients on disease processes, medications, and adherence.

Required Qualifications:
  • Active NP license or ability to obtain one by start date, free of restrictions or disciplinary actions.
  • National certification in Family, Geriatrics, Adult, or Adult-Gerontology primary or acute care, or ability to obtain certification/license in Colorado.
  • Physical ability to lift 30 pounds, navigate stairs, and perform physical assessments.
  • Reliable transportation to travel up to 85% within Colorado.
Preferred Qualifications:
  • At least 1 year of clinical experience in relevant settings.
  • Experience in Gerontology, Cardiology, Internal Medicine, or Endocrinology.
  • Home health or home visit experience.
  • Additional licensure in other regions may be requested.

Note: Employees must be in active regular status and remain in the role for at least 12 months to qualify for student loan repayment benefits, which require a 36-month commitment within Home and Community Care.

The salary range is $89,800 to $176,700 annually, based on full-time employment, experience, and other factors. Benefits include comprehensive packages, incentives, stock purchase options, and 401(k).
