This role requires travel across the entire state of Arizona, approximately 85% of the time. All travel expenses are covered, and there is a base salary plus incentives.

This role offers flexible scheduling with no on-call, weekends, or holidays. Travel up to 85% across Arizona, with all expenses covered, and competitive incentives.

Primary Responsibilities

• Conduct in-home assessments: medical history, review of systems, medication reconciliation, vital signs, physical exam
• Perform evidence-based screenings, including point of care testing
• Identify diagnoses for care management and treatment
• Formulate medical conditions based on assessments
• Communicate findings to inform the patient's PCP
• Identify urgent/emergent situations and intervene
• Educate members on health topics
• Travel up to 85%

Required Qualifications

• Active NP or PA license or ability to obtain by start date, with no disciplinary restrictions
• Active national NP or PA certification or ability to obtain before start date
• Physical ability to perform assessments and handle physical tasks
• Reliable transportation for home visits

Preferred Qualifications

• 1+ years of clinical experience
• Experience in Gerontology, Cardiology, Internal Medicine, or Endocrinology
• Home health or home visit experience
• Ability to travel within states for additional incentives

The salary range is $89,800 to $176,700 annually, based on experience and location. Benefits include comprehensive packages, incentives, stock purchase, and 401k.
