Job summary

UnitedHealth Group is seeking an NP/PA for in-home health assessments with Medicare Advantage members. This field-based, non-prescribing role emphasizes preventive care and patient education during home visits.

Flexible scheduling between 8 a.m. and 8 p.m., with no on-call, weekends, or holidays. You will work one-on-one with patients and coordinate with their primary care providers to close gaps in care.

Qualifications

• Active NP or PA license in good standing.
• Experience with in-home visits is preferred.
• Strong skills in preventive care and patient education.
