Responsibilities

• Provide high-quality outpatient primary care to adult patients in an internal medicine setting.
• Conduct patient evaluations, diagnose and manage acute and chronic conditions.
• Order and interpret diagnostic tests; develop and implement treatment plans.
• Provide preventive care and health maintenance counseling.
• Collaborate with faculty attending physicians, residents, and multidisciplinary care teams.
• Document patient care accurately in the electronic medical record.

TeamHealth is seeking an experienced advanced practice clinician to join our energetic outpatient internal medicine clinic in Colton, California. This is a full-time, primary care-focused role within an academic medical environment serving a diverse patient population.As nurse practitioner or physician assistant, you will provide comprehensive outpatient primary care services and will work closely with faculty attending physicians. This role offers strong clinical support, with readily available physician supervision and support for clinical questions, complex cases, and ongoing professional development.

Key Responsibilities

• Provide high-quality outpatient primary care to adult patients in an IM setting
• Conduct patient evaluations, diagnose and manage acute and chronic conditions
• Order and interpret diagnostic tests; develop and implement treatment plans
• Provide preventive care and health maintenance counseling
• Collaborate closely with faculty attending physicians, residents, and multidisciplinary care teams
• Document patient care accurately and timely in the electronic medical record
• Participate in quality improvement and population health initiatives as appropriate
