Riverside Home Health & Hospice is seeking a Hospice Nurse Practitioner to lead patient care across Southern Oregon, including Medford and Grants Pass. You will conduct hospice visits, assess patients, and partner with an interdisciplinary team to deliver comfort-focused, high-quality care.

• Graduate of an accredited NP program
• Current NP license in Oregon
• Strong clinical decision-making and assessment skills

• Valid driver’s license and reliable transportation
