Job summary

Transitions Hospice is seeking an Advanced Nurse Practitioner to lead clinical care with autonomy in a multidisciplinary team. You will guide patient plans, oversee care delivery, and ensure high-quality medical oversight across home, facility, and hospital settings.

The role emphasizes top-of-license practice, strong collaboration with physicians and staff, and travel within the assigned territory to support patient needs and effective care transitions.

Qualifications

  • Master’s degree in nursing and NP license required.
  • Active DEA license; controlled substances license as required by state.
  • 1+ year of hospice or home care experience.
  • Familiarity with hospice and palliative care principles.

Responsibilities

  • Provide direction and oversight for the Plan of Care and update it as patient needs evolve.
  • Complete and document all required Hospice Face-to-Face encounters.
  • Conduct routine visits focusing on symptom management, primary care, and quality of life.
  • Review and interpret diagnostic results in a timely manner.
  • Collaborate with attending physicians, specialists, home health, facilities, and coordinators for coordinated care.
  • Serve as emergency responder for on‑call visits to support the care team.
  • Maintain thorough clinical documentation in the EHR.

Education

Master’s Degree in Nursing
Unrestricted NP license in state of employment
Active DEA License
Hospice and Palliative Care (HPC) certification preferred

Salary Range: $100 per visit plus mileage

What You’ll Do

As a Transitions Hospice Advanced NP, you’ll serve as a clinical leader within our interdisciplinary care team — directing patient care, managing the Plan of Care, and ensuring every patient receives expert, compassionate medical oversight. Your responsibilities include:

  • Providing direction and oversight for the Plan of Care, implementing and updating it to meet each patient’s evolving medical needs
  • Completing and documenting all required Hospice Face-to-Face encounters
  • Conducting routine visits focused on symptom management, primary care, and quality of life
  • Reviewing and interpreting diagnostic results in a timely manner
  • Collaborating with attending physicians, specialists, home health, facilities, and clinical coordinators for seamless care coordination
  • Serving as emergency responder for on‑call visits as needed to support case managers and extended care team members
  • Maintaining thorough and accurate clinical documentation in the company EHR
  • Providing direct medical care in patients’ homes, nursing facilities, and hospital settings
  • Acting as a liaison between the hospice program, community physicians, and healthcare partners to strengthen relationships and expand patient access
  • Ensuring compliance with local, state, and federal laws, Medicare regulations, and HIPAA

Requirements
What You Bring
  • Master’s Degree in Nursing and current, unrestricted NP license in state of employment
  • Active DEA License with no restrictions; Controlled Substance license as required by state
  • 1+ year of hospice or home care experience
  • Strong working knowledge of hospice and palliative care, with emphasis on symptom management for terminal illness
  • Familiarity with geriatric medicine and end-of-life principles and philosophy
  • Ability to collaborate effectively within an interdisciplinary team and with community physicians
  • Comfortable using clinical equipment (BP monitor, pulse oximeter, otoscope, etc.)
  • Valid driver’s license and willingness to travel locally
  • Hospice and Palliative Care (HPC) certification preferred
