Transitions Hospice in Kalamazoo area seeks an Advanced Nurse Practitioner to lead clinical care within our interdisciplinary team. You will direct the Plan of Care, perform Hospice Face-to-Face encounters, and provide symptom management during home visits and in facilities.

Qualifications

• Master's Degree in Nursing and NP license in state of employment.
• Active DEA license and HPC certification if required.
• 1+ year hospice or home care experience.

• Willingness to travel locally and in-range communities.

Responsibilities

• Direct the Plan of Care and update it as patient needs evolve.
• Complete and document Hospice Face-to-Face encounters.
• Conduct routine visits for symptom management and quality of life.
• Interpret diagnostic results and coordinate care with physicians.
• Collaborate with home health, facilities, and care teams.
• Serve as on-call emergency responder to support case managers.
• Maintain accurate clinical documentation in the EHR.

Schedule: PRN as needed

Salary Range: $100 per visit plus mileage

What You'll Do

As a Transitions Hospice Advanced NP, you'll serve as a clinical leader within our interdisciplinary care team - directing patient care, managing the Plan of Care, and ensuring every patient receives expert, compassionate medical oversight. Your responsibilities include:

• Providing direction and oversight for the Plan of Care, implementing and updating it to meet each patient's evolving medical needs
• Completing and documenting all required Hospice Face-to-Face encounters
• Conducting routine visits focused on symptom management, primary care, and quality of life
• Reviewing and interpreting diagnostic results in a timely manner
• Collaborating with attending physicians, specialists, home health, facilities, and clinical coordinators for seamless care coordination
• Serving as emergency responder for on-call visits as needed to support case managers and extended care team members
• Maintaining thorough and accurate clinical documentation in the company EHR
• Providing direct medical care in patients' homes, nursing facilities, and hospital settings
• Acting as a liaison between the hospice program, community physicians, and healthcare partners to strengthen relationships and expand patient access
• Ensuring compliance with local, state, and federal laws, Medicare regulations, and HIPAA

Requirements

What You Bring

• Master's Degree in Nursing and current, unrestricted NP license in state of employment
• Active DEA License with no restrictions; Controlled Substance license as required by state
• 1+ year of hospice or home care experience
• Strong working knowledge of hospice and palliative care, with emphasis on symptom management for terminal illness
• Familiarity with geriatric medicine and end-of-life principles and philosophy
• Ability to collaborate effectively within an interdisciplinary team and with community physicians
• Comfortable using clinical equipment (BP monitor, pulse oximeter, otoscope, etc.)
• Valid driver's license and willingness to travel locally
• Hospice and Palliative Care (HPC) certification preferred
