Job summary

BAYADA Hospice is seeking a compassionate Per Diem Nurse Practitioner (NP) to join our team in Tucson, Arizona. The coverage area includes Tucson, with home-based hospice care and flexible per diem scheduling.

As a key member of a multidisciplinary team, you will perform patient assessments, hospice recertifications, and follow physicians' orders, traveling to cases as needed while utilizing EMR. An AZ NP license and at least 1 year of clinical experience are required; home care/hospice

Qualifications

• AZ NP license is required.
• Minimum of one year of recent clinical experience.
• Prior home care, hospice and/or palliative care experience preferred but not required.
• Proficient with EMR systems.
• Ability to travel to cases as assigned.
• Strong interpersonal skills.

Responsibilities

• Make home visits for hospice recertifications and patient assessments.
• Monitor clients' conditions and report changes to the interdisciplinary team.
• Follow up on and document doctors' orders accurately.
• Provide comprehensive patient assessments as needed.

BAYADA Hospice is hiring a compassionate and dedicated Per Diem Nurse Practitioner, (NP) to join our team. The coverage area for this position is in Tucson, Arizona.

Prior home care, hospice, or palliative care experience strongly preferred, but not required. As a hospice nurse NP, you will be an integral member of a multi-disciplinary health care team that provides palliative care to clients, affording them the opportunity to receive the medical care required to remain at home.

Nurse Practitioner (NP) Responsibilities:

• Making home face to face hospice recertification visits to clients in designated geographic territories.

• Monitoring clients' conditions; reporting changes to Clinical or Client Services Manager.

• Following up with, executing, and properly documenting doctors' orders.

• Performing client assessments as necessary.

Qualifications for a Nurse Practitioner (NP):

• A current license as a NP in AZ

• A minimum of one year of recent, verifiable clinical (medical/surgical) experience.

• Prior home care, hospice and/or palliative care experience strongly preferred, but not required.

• Solid computer skills; prior experience with electronic medical records (EMR) preferred.

• Ability to travel to cases as assigned.

• Strong interpersonal skills.
