Job summary

Daymark Health seeks a Nurse Practitioner to deliver hands-on oncology care in patients’ homes and via virtual platforms in San Diego. You’ll join a care team to implement our cancer care model and continuously improve processes that support high-quality patient outcomes.

As a player/coach, you’ll roll up your sleeves, triage high-acuity cases, coordinate with oncology partners, and build trusting relationships with patients and networks across the care continuum.

Qualifications

• Active NP or PA with 4+ years delivering direct clinical care.
• Oncology, home care, palliative or primary care experience required.
• Oncology Nursing Society certification a plus; value-based care experience a plus.

ABOUT THE ROLE

As one of Daymark's Nurse Practitioners, you'll work with our care team to implement our cancer care model with patients in their homes. Your primary goal is to provide hands-on, compassionate patient care directly in patients' homes and through virtual platforms. You'll also implement and help to continuously improve processes that will enable Daymark to be effective in San Diego, build a highly engaged patient population, and contribute to a high-performance clinical team. You must be comfortable operating as a player/coach, rolling up your sleeves, and doing whatever is required to provide high-quality clinical care and to grow quickly. You'll work closely with our clinical operations team, leveraging your expertise to establish our evidence-based oncology care and navigation model to address clinical (symptom management, in-home urgent care, palliative care, mental and behavioral health care) and non-clinical (social and related needs).

WHAT YOU'LL DO

• Provide clinical care: You'll provide direct patient care both in home and via virtual platforms. You'll conduct assessments, place orders, prescribe medications, document appropriately, and assign tasks to enable our broader care team to deliver great patient care.
• Enable Daymark's ability to triage for high acuity: You'll help us provide navigation and direct clinical care for our highest acuity patients while also supporting our broader patient population. You'll collaborate closely with oncology practices in our market through daily stand ups with their own clinical leadership teams.
• Support Patients Across the Full Care Continuum. Provide comprehensive clinical support from active treatment through survivorship or end-of-life care, adapting your approach to meet each patient where they are.
• Build Trusting Relationships. Establish strong, trusting relationships with patients and their care networks by prioritizing empathy and active listening in every interaction.
• Real-world application of a new care model: You'll collaborate with Daymark's clinical operations team to refine and implement our care model. You'll also help us deliver the right care at the right time to patients with a cancer diagnosis.

WHAT WE ARE LOOKING FOR

• Expertise: You are an active NP or PA and have been practicing for at least 4 years, delivering direct clinical care to patients. Oncology, home care, palliative care, hospice, or primary care experience required. Oncology Nursing Society certification, experience with value-based care and/or in-home care are each a plus.
• Organizational & Technology Skills. You excel at multitasking and thrive in fast-paced environments while maintaining meticulous organization in communications and documentation. You're comfortable using Slack, Zoom, Google Suite, and EHR systems.
• Analytical Skills. You can roll up your sleeves and contribute to clinical problem-solving.
• Bias to action. You know how to identify and prioritize your initiatives and ensure that urgent and important tasks always get done.
• Comfort with ambiguity. You have experience being a part of building or launching new programs and are comfortable with changing processes to improve patient care.
• Player/Coach. You can lead your team and can care for patients and switch contexts with ease, as all will be required in a day's work.
• Able to work in-person. You'll deliver care to patients in San Diego.
• Availability. You can work with our team and patients full-time, four days per week.

Additional Information

The expected compensation for this role is $140,000 to $170,000 annually, depending on location & experience.

This is a full time role operating on a 5 day / 8 hour schedule, Mon - Fri.

All Daymark Health patient-facing roles require Basic Life Support (BLS) certification and compliance with Daymark's Employee Vaccination and Testing policy. For roles that require in-home visits to patients, candidates must have a reliable personal vehicle and provide proof of a valid
