Daymark Health seeks a Nurse Practitioner to deliver hands-on oncology care in patients’ homes and via virtual platforms in San Diego. You’ll join a care team to implement our cancer care model and continuously improve processes that support high-quality patient outcomes.
As a player/coach, you’ll roll up your sleeves, triage high-acuity cases, coordinate with oncology partners, and build trusting relationships with patients and networks across the care continuum.
As one of Daymark's Nurse Practitioners, you'll work with our care team to implement our cancer care model with patients in their homes. Your primary goal is to provide hands-on, compassionate patient care directly in patients' homes and through virtual platforms. You'll also implement and help to continuously improve processes that will enable Daymark to be effective in San Diego, build a highly engaged patient population, and contribute to a high-performance clinical team. You must be comfortable operating as a player/coach, rolling up your sleeves, and doing whatever is required to provide high-quality clinical care and to grow quickly. You'll work closely with our clinical operations team, leveraging your expertise to establish our evidence-based oncology care and navigation model to address clinical (symptom management, in-home urgent care, palliative care, mental and behavioral health care) and non-clinical (social and related needs).
The expected compensation for this role is $140,000 to $170,000 annually, depending on location & experience.
This is a full time role operating on a 5 day / 8 hour schedule, Mon - Fri.
