Daymark Health is seeking a Nurse Practitioner to deliver hands-on oncology care in patients' homes and through virtual platforms in the Inland Empire region. The role combines direct clinical work with care coordination, supporting a growing value-based care model.

As one of Daymark's Nurse Practitioners, you'll work with our care team to implement our cancer care model with patients in their homes. Your primary goal is to provide hands‑on, compassionate patient care directly in patients' homes and through virtual platforms. You'll also implement and help to continuously improve processes that will enable Daymark to be effective in the Inland Empire region, build a highly engaged patient population, and contribute to a high‑performing clinical team. You must be comfortable operating as a player/coach, rolling up your sleeves, and doing whatever is required to provide high‑quality clinical care and to grow quickly. You'll work closely with our clinical operations team, leveraging your expertise to establish our evidence‑based oncology care and navigation model to address clinical (symptom management, in‑home urgent care, palliative care, mental and behavioral health care) and non‑clinical (social and related needs).
