Qualifications

• Family Nurse Practitioner (FNP) or other primary-care NP background required.
• Experience providing adult primary care.
• Experience treating patients living with HIV strongly preferred.
• Comfortable prescribing and managing medications.
• Strong chronic disease-management experience.
• Experience in community health, infectious disease, HIV medicine, behavioral health or substance-use treatment is desirable.
• Comfortable treating medically and behaviorally complex patients.
• Strong clinical assessment and decision-making skills.
• Comfortable working within a multidisciplinary care model.
• Interest in developing and growing a new clinical program.
• Experience serving underserved or high-risk patient populations is a plus.

Responsibilities

• Provide comprehensive primary care to adult patients.
• Evaluate, diagnose, and treat acute and chronic medical conditions.
• Provide ongoing care for patients living with HIV.
• Prescribe and manage medications within scope of practice.
• Monitor treatment effectiveness and adherence.
• Order and interpret labs and diagnostic tests.
• Provide preventive care and health education.
• Coordinate referrals to specialists and other providers.
• Maintain timely clinical documentation.
• Participate in quality and performance-improvement initiatives.
• Contribute to developing the new integrated healthcare initiative.
