Job description

Department: PCMG-MAIN | Hematology and Oncology. Location: Phoenix. Shift: Mon‑Fri, Days 8am‑5pm. Category: Advanced Practice Clinicians (AAP). Posting #: 1012218. Employee Type: Full‑Time.

Position Summary

This Advanced Practice Provider position will primarily care for the benign hematology patient population, providing outpatient care for both pediatric and adult patients. The provider may rotate onto the inpatient service in the future. Daily responsibilities include evaluating patients in clinic, ordering medications and blood products, managing outpatient prescriptions and home‑care orders, educating patients and families, coordinating with consultants, monitoring patient data, and scheduling follow‑up appointments. The provider will collaborate with other hematology team members and the broader CCBD to support project completion. Participation in quality or research initiatives is encouraged. The role is a 4‑day per week position with no weekend or holiday coverage.

Prior pediatric hematology experience is preferred, though not required.

This position works in collaboration with attending physicians, providing excellent clinical care within their scope of practice and playing a critical role in the patient family medical team. The provider may practice in hospital inpatient and/or outpatient settings.

Position Duties
  • Conduct comprehensive health history, physical assessments, and develop plans for infants, children, and young adults with acute or chronic health conditions.
  • Order and prescribe pharmacologic and non‑pharmacologic interventions; perform therapeutic or corrective measures to treat illness and improve health status.
  • Formulate family‑centered plans of care and maintain accurate documentation; collaborate with other team members.
  • Collect and analyze data when necessary.
  • Perform advanced procedures as required by credentialing (DOP).
  • Use evidence‑based practices to tailor care plans according to family needs and outcomes.
  • Serve as an educational resource for patients, families, nursing staff, and other health‑care teams.
  • Engage in professional development: maintain required competencies, renew certifications, pursue educational opportunities, and mentor trainees (Advanced Practice Provider students, residents, medical students).
  • Demonstrate commitment to patient safety by raising concerns appropriately, reporting through proper channels, and advocating practice change.
  • Carry out miscellaneous job‑related duties as requested.
Position Qualifications
Education
  • Master’s or Doctorate degree from a graduate nursing school accredited by an Accreditation Commission for Nursing (ACEN) or Commission on Collegiate Nursing Education (CCNE) in Pediatric Acute Care, Pediatric Primary Care, or Family Medicine.
Special Skills
  • Excellent interpersonal skills, strong verbal communication, and basic computer skills.
Additional Requirements
  • Current BLS certification for Healthcare Provider from the American Heart Association (or obtained by start date).
  • Current State of Arizona certification as a Nurse Practitioner (or obtained by start date).
