Nurse Practitioner – In-Home Health Risk Assessments (PRN)

Earn $125 per completed assessment with full mileage reimbursement

Are you a Nurse Practitioner looking for flexible, supplemental work that fits your lifestyle? We're hiring NPs to perform in-home Health Risk Assessments (HRAs) for Medicare Advantage members. Your Role:
As an NP, you’ll conduct comprehensive in-home HRAs for Medicare Advantage members. Each visit includes a review of medical history, medications, lifestyle factors, and social determinants of health. You'll also complete basic physical and mental health evaluations and provide preventive care education. No long-term care or follow-up is required.

Requirements:

  • Active Nurse Practitioner license (all specialties except PMHNP)
  • Ability to work a minimum of five full days (8 hours each) per month
  • Willingness to travel within your county and surrounding areas
