The Center for Healthy Senior Living Nurse Practitioner for subacute and inpatient care performs the role utilizing the patient-centered coordinated care model demonstrated through competencies such as leadership, direct clinical practice, consultation/collaboration, coaching/guiding, research, and ethical decision making. Preference for Geriatrics, Sub Acute, and Internal Medicine NP experience.
Responsibilities
- Leadership: Participates in decisions affecting APRN practice and in opportunities to advance nursing practice within the institution. Promotes HackensackUMC APRN practice recognition locally, nationally, and internationally through conferences, publications, and presentations. Provides excellence in patient experience through modeling Service Excellence Standards. Communicates a shared vision with the departments of patient care and specialty practice. Participates in APRN interviewing, credentialing, peer review, and membership in professional organizations. Supports the nursing blueprint with evidence of practice outcomes. Ensures compliance with nursing protocols, standards, and quality indicators. Provides 24/7 patient care coverage.
- Evidence-based Practice: Applies evidence and value-based practices to achieve positive patient outcomes. Evaluates care planning, completes medication reconciliations, and conducts peer reviews. Translates current protocols into practice, provides culturally appropriate education, and participates in interdisciplinary rounds. Develops evidence-based practice protocols, analyzes practice patterns, and serves as a clinical resource. Identifies practice gaps and initiates changes supporting evidence-based practices. Participates in policy updates.
- Consultation/Collaboration: Acts as a resource for clinical and systemic improvements. Participates in meetings and committees, builds credibility with medical staff, and collaborates on protocols. Mentors staff and students, and identifies educational needs for patients and families.
- Coaching/Guiding: Coaches staff on evidence-based care, participates in staff rounding, and facilitates educational programs. Promotes shared governance and a collaborative work environment.
- Research: Utilizes and disseminates research findings, promotes innovation, participates as a researcher or reviewer, and shares findings through presentations and publications.
- Ethical Decision-Making: Identifies ethical issues and coordinates interdisciplinary meetings to resolve them.
Qualifications
- Graduate of an NLN/AACN accredited nursing program.
- Master's degree required.
- Maintains clinical competencies through ongoing education and practice.
- Experience in relevant patient care settings or as regulated.
- Collaborative agreement with physicians as required.
Licenses and Certifications
- NJ State Professional Registered Nurse License.
- Advanced Practice Nurse License.
- AHA Basic Life Support HCP Certification.
- Nation or Board Certification in specialty area.
Starting Minimum Rate: Starting at $123,260.80 annually.
