Job summary

Rochester Regional Health's ElderONE program seeks a Nurse Practitioner to provide advanced primary care to older adults enrolled in PACE, delivering comprehensive, home- and community-based services.

You will collaborate with an interdisciplinary team, support participants to remain safely at home, and contribute to quality care within a highly engaged network. Monday–Friday, days, in ElderONE sites and community settings.

Qualifications

Must have RN with NP certification and be eligible for NYS licensure.
Experience with older adults and/or medically complex populations preferred.
Experience in PACE, Primary Care, Long-Term Care, Home Care, Geriatrics, or community-based care settings preferred.

Responsibilities

Clinical care: Evaluate and manage acute, sub-acute, and chronic needs of older adults.
Care management: complete comprehensive clinical reviews and document in medical record.
Attend daily interdisciplinary team meetings and care planning conferences.
Support ElderONE participants across sites and home settings with coordinated, person-centered care.
Contribute to program growth through collaboration and quality improvement activities.
Infection control: follow guidelines to minimize exposure to infectious diseases.
