Job summary

Rochester Regional Health’s ElderONE program seeks a Nurse Practitioner to provide in-home and community-based care for older adults in Rochester, NY. You will work with an integrated, interdisciplinary team to manage acute and chronic conditions, develop patient-centered plans, and support transitions of care.

The role emphasizes care coordination, quality of life, and staying within patients’ home and community settings.

Qualifications

MS in nursing is required.
Nurse Practitioner Certification (NY) is required.
RN license in New York State is required.
Experience with older adults or medically complex populations preferred.
Epic experience and PACE experience are a plus.

Responsibilities

Evaluate and manage acute, sub-acute, and chronic needs of older adults.
Collaborate with physicians and team to develop individualized treatment plans.
Complete comprehensive reviews of chronic conditions and coordinate follow-up care.
Attend daily team meetings and care planning conferences.
Support care transitions and discharge planning within PACE and community settings.
