Community Care, Inc. is seeking a Nurse Practitioner for a full-time, day-shift position serving the Racine/Kenosha and Milwaukee areas.

The role involves collaborating with an interdisciplinary team to deliver comprehensive care to elderly individuals and adults with disabilities in the community. The position offers a $10,000 sign-on bonus, with responsibilities including health history documentation, patient assessments, care planning, and health education for members and families.

Competenze

• Master's degree in Nursing required.
• Wisconsin RN license and NP certification required.
• 1 year clinical nursing experience with at-risk or elderly population.

Mansioni

• Documents health history, exams, functional and mental health evaluations, preventive care, and reminders.
• Leads interdisciplinary team, provides guidance and care planning assistance.
• Educates members and families about health maintenance and illnesses.
• Travel within local area using personal vehicle with valid license and insurance.
