Job summary

Premier Medical Group of the Hudson Valley is seeking a full-time Nurse Practitioner to join our Gastroenterology division in the Hudson Valley. This NP will provide comprehensive care in a multi-specialty practice, collaborating with physicians and staff to coordinate patient care.

The candidate should have an accredited NP program, NYS licensure, and 2–5 years of experience. Responsibilities include exams, ordering tests, prescribing therapies, and hospital rounds, with a Monday–Friday

Qualifications

  • Graduate of an accredited Nurse Practitioner Program; NYS licensed.
  • 2 – 5 years related experience; or equivalent combination of education and experience.

Responsibilities

  • Collaborate with physicians and health care providers to deliver care across the continuum.
  • Perform hospital rounds for new consults and follow-up patients and assist in discharge.
  • Obtain patient history and perform comprehensive health assessments.
  • Order and interpret diagnostic tests within protocol guidelines.
  • Prescribe and monitor pharmacologic and non-pharmacologic therapies.
  • Document patient care in EMR and ensure accurate coding.
  • Participate in quality assurance and performance improvement activities.
  • Provide patient education and coordinate referrals as needed.

JOB SUMMARY

Premier Medical Group is expanding and seeking a full time Nurse Practitioner to join our busy, multi‑specialty medical practice, located in the Hudson Valley. Premier is an independent practice, looking to enhance patient access with motivated and experienced clinicians. The ideal candidate will have a foundation of clinical experience, enthusiasm, and a patient‑centered approach.

This position includes the direct care of patients in coordination with our physicians and clinical support staff, comprehensive health assessment, management of complex disease states, ordering and review of laboratory and diagnostic tests, executing treatment plans, prescribing, authorizing, and monitoring medication regimens, and participating in hospital rounds. The NP will also consult with other departments, as appropriate, to provide an interdisciplinary approach to the patient’s needs.

ESSENTIAL DUTIES AND RESPONSIBILITIES
  • Works in collaboration with physicians and other health care providers to provide health care across the continuum of care to assigned patients.
  • Performs hospital rounds of new consults and follow-up patients and assists in discharge of patients.
  • Obtains patient medical, physical, and psycho‑social history and performs comprehensive health assessment including physical exam, based on age and history.
  • Conducts non-invasive preventive screening procedures based on age and history.
  • Identifies medical and health risks and needs. Updates and records changes in health status in EMR.
  • Formulates the appropriate differential diagnosis based on history, physical examination and clinical findings.
  • Orders and interprets appropriate diagnostic tests and laboratory studies within protocol guidelines.
  • Develops and provides a relevant patient education plan.
  • Prescribes and administers appropriate pharmacologic and non‑pharmacologic therapies.
  • Makes appropriate referrals to other health professionals and community agencies.
  • Determines the effectiveness of the plan of care through documentation of client care outcomes.
  • Reassesses and modifies the treatment plan as necessary to achieve medical and health goals.
  • Completes all documentation in EMR in a timely fashion. Codes patient visits accurately and appropriately.
  • Participates in quality assurance review on a periodic basis, including systematic review of records and treatment plans.
  • Treats patients and their families with respect and dignity.
  • onsults other departments, as needed and as appropriate, to provide an interdisciplinary approach to the patient’s needs.
EDUCATION & EXPERIENCE
  • Graduate of an accredited Nurse Practitioner Program; NYS licensed.
  • 2 – 5 years related experience; or equivalent combination of education and experience.
