Job summary
OnePeak Medical seeks a Nurse Practitioner to join our Primary Care Functional Medicine team in Talent, Oregon. You will deliver comprehensive medical services, conduct office visits and telehealth, and collaborate with a high-functioning care team to drive root-cause health outcomes.
Relocation offered, no nights or weekends, a four-day work week, strong clinical support, mentorship, and comprehensive benefits.
Qualifications
- Active Oregon NP license or eligible for licensure.
- Board certification in Family Medicine or Internal Medicine.
- Strong interest in preventive and lifestyle medicine.
- Excellent communication and patient relationship skills.
- Experience with NextGen EHR is a plus.
Responsibilities
- Provide direct patient care via office visits and telehealth.
- Perform comprehensive assessments and diagnose conditions.
- Develop personalized treatment plans including lifestyle interventions.
- Educate patients on preventive health and functional medicine.
- Collaborate with health coaches, nutritionists, and mental health staff.
- Document visits in NextGen EHR in a timely, compliant manner.
- Participate in team meetings and quality improvement initiatives.
- Support clinic operations and foster a positive team environment.
