Job summary

Essen Health Care in New York seeks a Nurse Practitioner to provide advanced primary and/or specialty care to patients across our multispecialty network. You will perform physical exams, take medical histories, and develop care plans.

Responsibilities include conducting exams, ordering tests, diagnosing conditions, and coordinating with subspecialists, with opportunities across primary care, urgent care, nursing homes, and house calls.

Qualifications

• Master's Degree in a Nurse Practitioner program (Family or Adult Geriatrics).
• Current New York State License as a Nurse Practitioner.
• National Board Certification required.
• AANP certification.
• ANCC certification.
• ONCC certification.

Responsibilities

• Examines, diagnoses and treats patients.
• Ensures fulfillment of Essen Health Care commitment for ensuring quality care.
• Diagnoses and treats variety of diseases in general practice.
• Collects, records and maintains accurate patient information.
• Documents past medical, family and social history, exams, medications, allergies, assessment and plan.
• Orders or conducts tests and diagnostic imaging to inform treatment.
• Analyzes test results and examination findings to diagnose conditions.
• Administers or prescribes treatments.
• Orders lab and imaging tests to manage an immediate treatment plan.
• Vaccinates patients to immunize against diseases.
• Advises patients on diet, hygiene, and prevention of disease.
• Consults with appropriate medical subspecialists within the network.

• Orders lab and imaging tests to determine and manage an immediate treatment plan
• Vaccinates patients to immunize patients from communicable diseases
• Advises patients concerning diet, hygiene, and methods for prevention of disease
• Consult with appropriate medical specialist when necessary; refer patient to subspecialists within network
