Job summary

Pyramid Healthcare Langhorne seeks a Nurse Practitioner to provide medical and psychiatric evaluation, medication management, and care coordination for adults with substance use and mental health needs. You will collaborate with interdisciplinary teams to deliver high-quality, person-centered care.

This role requires an active NP license, PA practice, and DEA registration, with preference for board certification. The in-person shift options include standard daytime hours in Langhorne, PA.

Qualifications

  • Active and unencumbered Nurse Practitioner/CRNP license in the state of practice
  • DEA registration required or eligible to obtain if prescriptive authority applies
  • Background in behavioral health, substance use disorder treatment, and co-occurring disorders preferred
  • Board Certification preferred

Responsibilities

  • Complete history and physical assessments and related documentation.
  • Review labs, monitor levels, and support medication management.
  • Provide medication management services including psychotropic meds where applicable.
  • Diagnose and treat medical/psychiatric conditions within scope of practice.
  • Coordinate client care with internal and external healthcare providers.
  • Support discharge planning and continuity of care, including aftercare recommendations.
  • Collaborate with nursing and clinical teams on cost-effective med management.

The Nurse Practitioner provides medical and/or psychiatric evaluation, assessment, medication management, care coordination, and ongoing treatment services for individuals receiving behavioral health and substance use disorder treatment services. This role collaborates with interdisciplinary treatment teams and internal and external healthcare providers to support high-quality, person-centered care in accordance with organizational, regulatory, and professional practice standards.

(In-Person) Monday - Friday 7:00am - 3:00pm, 8:00am - 4:00pm, or 9:00am - 5:00pm On-Call via phone or virtually

Education, Experience
  • Active and unencumbered Nurse Practitioner/CRNP license in the state of Pennsylvania
  • Active license to practice in General/Family Medicine
  • DEA registration required or eligible to obtain if prescriptive authority applies
  • Facilities with specific populations may prefer candidates with specialized backgrounds, including family practice, psychiatry, substance use disorder treatment, and/or co-occurring disorders
  • Board Certification preferred
  • Strong background in behavioral health, substance use disorder treatment, and co-occurring disorders preferred
Essential Duties And Responsibilities
  • Complete history and physical assessments, evaluations, reassessments, and other clinical documentation as required.
  • Review laboratory results, monitor blood levels, review physician orders, and support ongoing medical and medication management needs.
  • Provide medication management services, including psychotropic medication management where applicable based on licensure and scope of practice.
  • Diagnose and treat medical and/or psychiatric conditions in accordance with scope of practice and applicable regulations.
  • Coordinate client care with internal and external healthcare providers, including primary care physicians, psychiatrists, specialists, pharmacies, and other treatment providers as appropriate.
  • Support discharge planning and continuity of care efforts, including medication coordination and aftercare recommendations.
  • Collaborate with nursing and clinical teams to support cost-effective medication management and completion of medication prior authorizations when needed.
  • Provide education and clinical support to staff regarding medical, psychiatric, behavioral health, and medication-related concerns as appropriate.
  • Participate in treatment team meetings, clinical review meetings, peer review processes, and departmental meetings as required.
  • Maintain timely, accurate, and compliant documentation in accordance with CARF standards, licensing requirements, payer requirements, and organizational policies.
  • Must exercise discretion and maintain confidentiality with regard to all company information
  • Completion of all required trainings as designated by the company and accreditation/licensing entities.
  • Other duties as assigned.

Physical Demands

While performing the duties of this job, the employee is frequently required to talk or hear. The employee is regularly required to sit, use hands to handle or feel objects, and reach with hands and arms. The employee may occasionally climb stairs, stand, walk, stoop, twist, kneel, or crouch. The employee may occasionally lift or move up to 25 pounds and seldom up to 50 pounds. Specific vision abilities required include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.

Work Environment

This position operates in clinical, healthcare, office, residential, and behavioral health treatment environments. Local travel may be required based on operational needs, and employees may be required to provide their own transportation. Employees may interact with individuals experiencing psychiatric, behavioral health, substance use, or crisis-related situations and may encounter exposure to viruses, diseases, infections, and emotionally sensitive or traumatic situations during the course of providing services.
