Job summary

Pyramid Healthcare is seeking a Clinical Assessor II to deliver a full spectrum of clinical services in mobile and community settings across Pennsylvania.

You will perform assessments, develop treatment plans, and coordinate discharge and aftercare with teams, referral sources, and families, ensuring compliance with ASAM and regulatory standards.

Qualifications

ASAM-aligned substance programs require a Master’s with licensure or CADC certification.
Bachelor’s with CADC or pursuing CADC and 1 year clinical experience.
Associate’s with CAAC or pursuing CAAC with 2 years clinical experience.
High School Diploma or equivalent with CAAC.
Current Commonwealth RN licensure and nursing degree with 1 year clinical experience.
ASAM-aligned Co-Occurring enhanced programs require Master’s degree and licensure or CADC pathway.

Responsibilities

Provide clinical services including individual, family, and group sessions with referrals as needed.
Conduct mobile behavioral health and SUD assessments in assigned areas.
Complete biopsychosocial assessments, intakes, and reassessments.
Assess clients’ behavioral, emotional, cognitive, social, and recovery needs.
Perform ASAM assessments and determine levels of care per standards.
Develop admission, treatment, and clinical recommendations.
Create individualized treatment plans with clients and treatment team.
Collaborate with referral sources, hospitals, and community partners on planning.
Support service planning and clinical care needs with treatment teams.
Facilitate utilization reviews and maintain compliant documentation.
Support aftercare planning, referrals, and continuity of care.
Maintain confidentiality and professional boundaries.
Participate in on-call rotations as assigned.

This role will support the Pennsylvania Clinical Assessment Team and applicants are required to reside and be licensed in the state of Pennsylvania.

Summary

The Clinical Assessor II provides a range of clinical services in accordance with program requirements, applicable regulations, accreditation standards, and organizational policies. Responsibilities include assessment, treatment planning, counseling, case management, crisis intervention, referral coordination, discharge planning, group facilitation, and other assigned clinical services.

This role collaborates with clients, families, referral sources, community partners, and interdisciplinary treatment team members to support engagement, continuity of care, and positive treatment outcomes.

Essential Duties And Responsibilities

Provide clinical services, including individual, family, and group sessions, and maintain communication with referral sources and outpatient providers as needed.
Conduct mobile behavioral health and substance use disorder assessments within assigned service areas.
Complete biopsychosocial assessments, clinical evaluations, intakes, and reassessments.
Assess clients’ behavioral, emotional, cognitive, social, environmental, and recovery support needs.
Perform ASAM assessments and determine appropriate levels of care in compliance with credentials, licensing standards, and regulations.
Develop admission, treatment, and clinical recommendations based on assessment findings.
Complete individualized treatment plans in collaboration with clients and the treatment team.
Collaborate with referral sources, hospitals, emergency departments, providers, families, and community partners regarding treatment and admission planning.
Partner with treatment teams to support service planning and clinical care needs.
Facilitate utilization review processes and maintain documentation to support authorizations and continued stay reviews.
Support aftercare planning, referrals, and continuity of care coordination.
Maintain timely, accurate, and compliant documentation in accordance with regulatory and organizational standards.
Participate in on-call rotations as assigned.
Maintain confidentiality and exercise appropriate professional discretion.
Complete all required trainings per organizational, licensing, and accreditation requirements.
Perform other duties as assigned.

Required Qualifications

Licensure, Education, & Experience

ASAM aligned substance programs must meet one of the below requirements:
- Master’s Degree from an accredited college with a major in chemical dependency, psychology, social work, counseling with a Master’s level licensure: Marriage and family therapy, social work, professional counseling, or certification by a state wide certification board (CAADC).
- Master’s Degree from an accredited college with a major in chemical dependency, psychology, social work, counseling and are working towards a Master’s level licensure (Marriage and family therapy, social work, professional counseling), or certification by a state wide certification board (CAADC).
Bachelors level with a CADC or are working towards a CADC with a minimum of one year of qualifying clinical experience.
Associate's level with a CAAC or working towards a CAAC with a minimum of two years of qualifying clinical experience.
High School Diploma or equivalent with a CAAC.
Current licensure in this Commonwealth as a registered nurse and a degree from an accredited school of nursing and one year of qualifying clinical experience.
ASAM aligned Co-Occurring enhanced programs must meet one of the below requirements:
- Master’s Degree from an accredited college with a major in chemical dependency, psychology, social work, counseling with a Master’s level licensure: Marriage and family therapy, social work, professional counseling.

Physical Demands

While performing the duties of this job, the employee is frequently required to talk or hear. The employee is regularly required to sit, drive, use hands to handle or feel objects, and reach with hands and arms. Work Environment

This position operates primarily in mobile and community-based environments and requires frequent travel throughout assigned service areas. Assessments and admissions activities may occur in hospitals, emergency departments, residential facilities, outpatient programs, correctional facilities, schools, community agencies, client residences, and other referral locations. Employees may interact with individuals experiencing psychiatric, behavioral health, substance use, withdrawal, or crisis-related situations and may encounter exposure to viruses, diseases, infections, and emotionally sensitive or traumatic situations.
