Baptist Primary Care Lane in Jacksonville, FL is seeking a full-time Nurse Practitioner focused on Family Practice to join our team. The provider will care for Women’s and Men’s health, pediatric care, acute and chronic patients, perform follow-ups, and support wellness exams, using Epic EMR.

The role features 40 patient-facing hours per week with no on-call schedule; requires a Master’s degree, 2+ years in Family Practice, and current APRN licensure.

Qualifications

• Master's degree required and active APRN licensure.
• 2+ years of Family Practice experience.
• Certified and Licensed APRN, local candidates preferred.

Responsabilités

• Provide family practice care for women, men, and children.
• Manage acute and chronic patient conditions with follow-ups.
• Coordinate physician-patient follow-ups and hospital follow-ups; conduct wellness exams.
