Job summary

Primary Care Solutions is seeking a skilled Nurse Practitioner to provide float coverage at VA Community-Based Outpatient Clinics. You will work across states where PCS operates, delivering high-quality care to US Veterans and collaborating with the Patient Aligned Care Team to ensure standards of care.

This position offers a hybrid schedule with two weeks onsite and two weeks remote, competitive pay, comprehensive benefits, and CME opportunities.

Qualifications

• Graduate of an accredited school of nursing and MSN program.
• Board-certified in family practice or internal medicine.
• DEA registration current and valid.
• BLS certification for providers, renewed annually.

Responsibilities

• Examine patients and determine best course of action, documenting in The VA's VISTA/CPRS system.
• Develop and implement patient management plans with PACT collaboration.
• Discuss patient outcomes, policies, procedures, and records with Medical Director.
• Complete clinical reminders to keep each patient up to date in the system.

RESPONSIBILITIES:

As aNurse Practitioner with Primary Care Solutions, you'll have the opportunity to serve our nation's Veterans with the same exceptional level of care our Veterans have shown while serving our country. We are seeking healthcare professionals who are driven and committed to ensuring the health and well-being of our nation's Veterans.

• Examine the patient/client and determine the best course of action while documenting in our paperless health program The VAs VISTA/CPRS electronic patient record system.
• Develop and implement patient management plans, communicating with Patient Aligned Care Team (PACT) to ensure appropriate standards of care.
• Discuss patient outcomes, policies, procedures, and records with Medical Director.
• Completes all clinical reminders to keep each patient up to date in the system.

JOB REQUIREMENTS:

• Graduate of an accredited school of nursing and graduate of an accredited program for nurse practitioners (MSN required).
• Board-certified in family practice or internal medicine
• Certification by an applicable professional organization
• 1 year of clinical medical practice preferred
• Multiple active state licensure and DEA; maintain credentials at multiple VAMCwhere PCS operates
• Current BLS for providers from the American Heart Association (to be renewed annually)
• Current, unrestricted Drug Enforcement Administration (DEA) registration
• Excellent computer skills
