Primary Care Solutions is seeking a Nurse Practitioner to provide float coverage at VA Community-Based Outpatient Clinics across states we operate. You will deliver high-quality care to US Veterans, supporting the Patient Aligned Care Team and coordinating with Medical Director.

The role features a hybrid work arrangement with two weeks onsite and two weeks remote, standard clinic hours, and comprehensive benefits.

Qualifications

• Graduate of an accredited school of nursing and MSN required.
• Board-certified in family practice or internal medicine.
• Certification by an applicable professional organization.
• 1 year of clinical medical practice preferred.
• Multiple active state licensure and DEA; maintain credentials at multiple VAMC where PCS operates.
• Current BLS for providers from the American Heart Association (to be renewed annually).
• Current, unrestricted DEA registration.
• Excellent computer skills.

Responsibilities

• Examine the patient/client and determine the best course of action while documenting in our paperless health program The VAs VISTA/CPRS electronic patient record system.
• Develop and implement patient management plans, communicating with Patient Aligned Care Team (PACT) to ensure appropriate standards of care.
• Discuss patient outcomes, policies, procedures, and records with Medical Director.
• Completes all clinical reminders to keep each patient up to date in the system.
