Advocare LLC Nurse Practitioner role in New Jersey delivers comprehensive, patient-centered healthcare across the lifespan within a collaborative clinical team.

The NP conducts physical exams, orders and interprets labs, and prescribes medications as appropriate. You will educate patients, develop care plans, and coordinate care transitions with physicians to ensure high-quality outcomes.

Qualifications

• Board-certified Advanced Practice Nurse licensure required.
• Graduate degree from an accredited nursing program (MSN).
• Experience delivering patient-centered care across the lifespan.
• Ability to order/interpret labs and prescribe medications within scope.

Responsibilities

• Perform physical examinations to evaluate health and diagnose illnesses.
• Order and interpret labs and diagnostic tests.
• Create and contribute to patient care plans.
• Modify treatments based on patient changes.
• Educate patients and families on health problems and home care.
• Coordinate transitions between care settings (admission, transfer, discharge).
• Collaborate with multidisciplinary team and refer as needed.
• Plan and manage patient care according to individual needs.
• Ensure compliance with HIPAA, OSHA and other regulations.
• Maintain knowledge of current medical research and trends.
