Job summary

Community Health Center of the North Country (CHCNC) in Malone, NY, is hiring for an Advanced Practice Provider (Physician Assistant or Nurse Practitioner) to join our outpatient team. You will deliver high-quality primary care across ages, diagnose illnesses, manage chronic conditions, and coordinate care with supervising physicians.

We offer a full-time, permanent position with daytime hours, in-person work, a competitive annual salary, and a negotiable signing bonus.

Qualifications

• Current, unrestricted New York State PA or NP license.
• Current, unrestricted DEA registration.
• Current Board certification.
• Current BLS certification.
• Ability to work collaboratively with other healthcare professionals.
• Strong commitment to compassionate, patient-centered care.
• Minimum two years of PA/NP experience.

Responsibilities

• Assess and institute appropriate care within established primary care guidelines.
• Conduct initial examinations with treatment and follow-up recommendations.
• Order routine and diagnostic lab and x-ray studies.
• Diagnose and manage acute and chronic illness.
• Formulate and monitor patient care plans with supervising physician.
• Review and maintain patient records to required standards.
• Prescribe medications within licensure scope.
• Ensure referrals and consultations with collaborating physicians when needed.
• Maintain proficiency in using eCW EMR for documentation.
