Nurse Practitioner - Family Practice - $105K-150K per year

ChenMed is seeking a Nurse Practitioner specializing in Family Practice for a position in Houston, Texas.

Job Overview

The Nurse Practitioner (NP) will provide direct patient care in various settings including ChenMed/Jencare medical centers, nursing homes, skilled nursing facilities (SNF), and home care, depending on assignment. Responsibilities include geriatric assessments, medical histories, physical exams, diagnosis, treatment, care planning, health education, referrals, case management, and documentation according to ChenMed standards.

Key Responsibilities
  1. Assess acute and non-acute clinical problems independently.
  2. Develop and implement patient management plans.
  3. Order and interpret laboratory and diagnostic tests.
  4. Prescribe medications and treatments within scope.
  5. Refer patients to specialists as needed.
  6. Participate in clinical rounds and conferences.
  7. Recognize urgent situations and initiate life-saving procedures.
  8. Collaborate with healthcare team members to ensure quality care.
  9. Document patient interactions thoroughly.
Qualifications
  • ARNP or equivalent advanced nursing degree.
  • Current Nurse Practitioner certification in the state of practice.
  • Board certification by AANP or ANCC preferred.
  • Minimum 1 year of clinical experience preferred; strong new graduates considered.
  • BLS certification required within first 90 days.
