Job summary

LifeBridge Health is seeking a qualified Nurse Practitioner to diagnose and manage acute, chronic, and preventive health needs. You will prescribe medications, including controlled substances, and coordinate referrals with physicians and specialists to ensure comprehensive patient care.

Maintain meticulous documentation and collaborate with an interdisciplinary team to monitor patient progress, respond to emergencies, and mentor staff in a values-driven, patient-centered environment.

Qualifications

• Maryland CRNP license is required.
• Maryland CDS license or eligibility to obtain licensure is required.
• Active DEA Controlled Substance Registration is required.
• Current Basic Life Support (BLS) certification is required.

Responsibilities

• Diagnose and manage acute, chronic, and preventive healthcare needs through comprehensive patient assessments, examinations, and treatment planning.
• Prescribe medications, including controlled substances when appropriate, and collaborate with physicians and specialists for referrals and complex care needs.
• Maintain accurate, complete, and timely medical documentation in compliance with regulatory requirements, organizational policies, and quality standards.
• Collaborate effectively with physicians, nurses, and interdisciplinary team members to coordinate care, monitor patient progress, and advocate for patient needs.
• Respond to patient concerns, emergencies, and crises using established protocols while providing medical guidance in person and by telephone as needed.
• Demonstrate leadership through mentoring staff, participating in team meetings and educational programs, maintaining clinical competency, and supporting a culture of teamwork, compliance, and patient-centered care.

Education
