Job summary

Lexington Health in Columbia, SC seeks a full-time Nurse Practitioner/RN practitioner to assess patients, develop care plans, and collaborate with physicians across the patient care continuum.

Responsibilities include health history, exams, orders, education, and follow-up. Full-time, day shift with on-call duties and comprehensive benefits are provided by the Lexington Health network.

Qualifications

• Current licensure as RN in the State of South Carolina
• Certification by appropriate credentialing organization recognized by South Carolina as Nurse Practitioner
• CPR, BLS.

Responsibilities

• Documents all situations and maintains accurate patient charts and records.
• Provides diagnosis and treatment of common acute illnesses and preventive care for patients with stable chronic conditions.
• Performs history taking, physical exams, orders diagnostics, and develops treatment plans with supervising physicians.
• Maintains patient education and adherence to care plans, including lifestyle and preventive guidance.
• Reviews problem lists and updates medication lists at annual or episodic visits.
