Qualifications

Responsibilities

• Assess patients' physical and psychosocial status via interview, history, exam, and tests.
• Develop and implement treatment plans and follow patient care across the continuum.
• Provide health education and anticipatory guidance to patients and families.
• Maintain accurate records and medications lists for administrative purposes.
