Job summary

Community Health & Wellness Center (CHWC) in Connecticut seeks an experienced NP to provide clinical care within the NP scope. Active APRN license, CPR certification, and a master’s degree from an accredited program are required; CHWC prefers Family NP with Primary Care experience.

You will work with the clinical team to assess, diagnose, and manage patient care, collaborate with physicians, and participate in community health programs.

Qualifications

• Current APRN license is required.
• CPR certification required.
• Master’s degree from an accredited program in nursing.

Responsibilities

• Provides clinical care within scope of Nurse Practitioner license.
• Assesses patient illness and records medical history.
• Diagnoses illness or health needs and manages patient care plans.
• Collaborates with physicians and other staff to ensure quality patient care.
• May concentrate in medical specialties with appropriate board certifications.
