Job summary

Community Health and Wellness of Greater Torrington, Inc. is seeking an experienced Advanced Practice Registered Nurse to provide patient care within the APRN scope.

You will collaborate with physicians and the clinical team to manage patient plans and outcomes, ensuring high-quality care across primary care and community health programs. Required are an active APRN license, MSN and certified NP program completion, and experience in primary care settings; pediatric experience is a plus.

Qualifications

• Current APRN license.
• CPR certification.
• Master’s degree in nursing and completion of an accredited certified nurse practitioner program.
• Experience as a nurse practitioner in a Community Health Center/Primary Care setting or ER/Urgent Care.
• Family NP training preferred with Pediatric experience.

Responsibilities

• Provides clinical care within the scope of Nurse Practitioner license.
• Assesses patient illness by taking complete medical history and performing physical examinations.
• Diagnoses illness or health needs based on clinical data and assessments.
• Provides direct patient care as part of the clinical team with Nurses, MDs, Medical Assistants, Case Managers and Patient Access Representatives.
• Collaborates with team to ensure quality patient care and efficient workflow.
• Concentrates in specialties appropriate to skills and board certifications.
