Job summary

Heartland Health Services is seeking a Nurse Practitioner or Physician Assistant for a full-time day shift at Sheridan and Wisconsin site locations. The role offers an annual salary of $110,000–$125,000, with perks including PTO, 401K, and day-one health benefits.

Responsibilities include patient assessment, care planning, diagnostics, treatment, documentation, and collaboration with physicians to promote preventive care and manage acute and chronic conditions.

Qualifications

• Must be a licensed NP or PA with current state license (and DEA).
• 1-2 years’ NP/PA experience required.

Responsibilities

• Assess patients with history and exam to identify problems and needs.
• Plan and implement care, order tests, and educate patients.
• Conduct and interpret tests, prescribe therapies, and refer as needed.
• Document outcomes, reassess plans, and maintain records.
• Participate in quality assurance and team collaborations.
• Support continuous quality improvement and compliance.
