Responsibilities

The Nurse Practitioner, Clinical Triage, acts as the primary provider and clinical contact for patients requiring evaluation for issues needing further intervention. The role involves acting as a liaison between triage teams and local clinical teams.

Essential Duties and Responsibilities

• Serve as a clinical resource for triage teams
• Evaluate and treat patients via telehealth for acute needs
• Coordinate follow-up care with local teams
• Manage time-sensitive medication refills and lab results
• Interact with primary care providers for escalation or evaluation
• Address urgent messages and assist in service recovery
• Collaborate with pharmacy for comprehensive care
• Sign orders such as home health, DME, and death certificates
• Review quality metrics and identify improvement areas
• Educate patients on the Urgent Care Triage program
• Identify care gaps and coordinate external resources
• Work with leadership to improve care
• Perform additional duties as assigned

This is a remote role with working hours from 8 AM to 8 PM on Saturday, Sunday, and Monday.

Qualifications

Required

• Certified Nurse Practitioner with active licenses in coverage states
• Active DEA license
• Excellent communication skills
• Ability to react decisively in urgent situations
• Experience with multi-tasking and organization

Preferred

• 1 year of experience at HarmonyCares Medical Group
• Experience with APRIMA EMR
• Experience with telehealth and virtual care coordination
• Multistate License (MSL)
