Job description

What you’ll get to do in this role:

• Provide education to patient and/or the patient’s family based on the patient’s condition and identify needs to optimize understanding. Provide health maintenance and disease management counseling to patients.
• Obtain comprehensive medical histories, perform physical examinations/assessments, and initiate admission orders as appropriate.
• Provide medical management and/or other interventions for acute and chronic health problems.
• Implement a plan of care and perform procedures according to the Advanced Practitioner’s education, training, experience and competencies. Refer patients to specialists and/or therapists when appropriate and in a timely manner to ensure continuity of care.
• Order and interpret lab studies and diagnostic tests to evaluate the effectiveness of treatment as appropriate. Notify patients promptly of test results. Collaborate with physicians, as necessary, to verify diagnosis, identify high-risk situations, or alert to changes in patient’s status and take appropriate action.
• Write orders for prescription medications under the established protocol.
• Serve as a liaison and a resource for physicians, health care team members, patients and families.
• Participate in plans for health promotion, prevention and maintenance for the organization and the community.
