Job summary

Arizona Kidney Disease & Hypertension Center (AKDHC) in Phoenix is seeking a Nurse Practitioner to join our team. You’ll support individuals living with renal disease using your clinical expertise and patient‑centered approach.

Join a fast‑paced clinical environment with nephrologists and dialysis teams, manage CKD patients, coordinate care across sites, prescribe within scope, and help improve outcomes while growing professionally.

Qualifications

  • Master’s degree in nursing or related field (4–10 years of experience optional).
  • Current Arizona RN license with NP or PA license.
  • Proficiency in Microsoft Office and experience with ICD-9 and CPT coding.
  • Strong organizational skills, flexible schedule, and reliable transportation for travel between sites.
  • Patient-first mindset and commitment to compassionate care.

Responsibilities

  • Partner with nephrologists, dialysis teams, and interdisciplinary care providers to deliver comprehensive dialysis care, address patient concerns, and ensure accurate, timely clinical documentation.
  • Manage chronic kidney disease patients across outpatient settings by conducting assessments, developing treatment plans, and coordinating ongoing care in collaboration with physicians.
  • Facilitate seamless transitions of care through annual wellness visits, post-hospital follow-up, medication reconciliation, and patient education to improve outcomes and reduce readmissions.
  • Provide advanced clinical care across multiple settings—including physician offices, skilled nursing facilities, rehabilitation centers, dialysis units, and on-call environments—through diagnostic evaluations, treatment planning, and evidence-based interventions.
  • Prescribe medications and therapeutic treatments within scope of practice while delivering compassionate, patient-centered care and maintaining the highest standards of professionalism and clinical excellence.

You’ll make an impact by:
