Base pay range

$145,000.00/yr - $165,000.00/yr

The Role

As a Nurse Practitioner, you'll provide exceptional care to senior Medicare patients, addressing their needs with a holistic approach. Responsibilities include:

1. Supporting complex patients navigating the healthcare system
2. Providing patient education, creating care plans, and guiding the care team
3. Collaborating with DaVita's Integrated Care team for dialysis patients
4. Working autonomously and with the healthcare team to deliver high-quality, cost-effective care
5. Coordinating care among patients, families, physicians, and providers
6. Administering tests and screenings
7. Performing comprehensive health assessments and documenting accurately
8. Maintaining confidentiality per HIPAA regulations

What You Can Expect

This full-time role involves in-clinic and in-home visits within your service area, with occasional telehealth. Flexibility across regions may be required. We seek candidates passionate about quality care, patient education, and holistic health.

Qualifications

• California Nurse Practitioner license
• Board Certified NP in good standing
• NP certification from ANCC / AANP
• 2+ years clinical experience in Primary Care, Family Medicine, Internal Medicine, Gerontology, or Palliative Care; home care experience is a plus
• Knowledge of chronic care management and medications
• DEA license / Prescriptive Authority
• CPR/BLS Certification
• Experience with HCCs
• California Driver’s License
