Nurse Practitioner (Board-Certified Psychiatric APRN)

Hybrid — telepsychiatry from an approved Hawaii-based home office and/or in-person clinical care at OhanaPsych's Honolulu office. Schedule mix determined by clinical need and assignment.

Position Summary

You will provide direct psychiatric care to children, adolescents, and/or adults across Hawaii in a hybrid model: a high-volume telepsychiatry practice paired with in-person clinical care at Ohana Psych sites. You will carry a defined panel, manage acute and chronic psychiatric conditions, prescribe and monitor psychotropic medications including controlled substances, and coordinate care with primary care, therapy, and community resources. Strong autonomy, structured workflows, and full back-office support let you focus on patient care.

About Ohana

Ohana Psych is a clinician-led behavioral health organization serving patients across the Hawaiian Islands, primarily through telepsychiatry with in-person care as clinically indicated. The organization is part of a broader Ohana group expanding rural access to integrated mental health and primary care in Hawaii.

  • Conduct comprehensive psychiatric evaluations, diagnostic assessments, and treatment planning for assigned patients via telepsychiatry and in person.
  • Prescribe, titrate, and monitor psychotropic medications, including Schedule II-V controlled substances, in accordance with Hawaii and federal law.
  • Provide medication management follow-up visits, brief supportive psychotherapy when clinically indicated, and crisis assessment as needed.
  • Perform in-person clinical activities as assigned, which may include physical examination components relevant to psychiatric care (e.g., AIMS), in-office injectable administration (e.g., long-acting injectable antipsychotics), and direct patient assessment.
  • Document all patient encounters in the electronic health record within established timeliness standards (typically same-day close).
  • Submit complete, accurate charge information to support timely billing and clean-claim performance.
  • Coordinate care with primary care providers, therapists, schools, hospitals, and community agencies; respond to clinical messages and refills within posted turnaround times.
  • Maintain compliance with credentialing, payer enrollment (Medicare, Med-QUEST, HMSA, AlohaCare, 'Ohana/WellCare, UHC, commercial), CAQH attestation, and DEA registration.
  • Participate in peer review, case consultation, quality improvement, and continuing medical education activities.
  • Comply with all Ohana policies, HIPAA, 42 CFR Part 2 (where applicable), and state and federal regulations governing the practice of nursing and prescribing.
  • Performs other duties as assigned consistent with the role and within scope of practice.
Required Qualifications
Education

Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited psychiatric mental health nurse practitioner program.

Licensure / Certification
  • Current unrestricted APRN license with prescriptive authority in the State of Hawaii (required prior to start; multi-state RN licensure acceptable as a base).
  • Board certification as a Psychiatric-Mental Health Nurse Practitioner (PMHNP-BC) through ANCC.
  • Current, unrestricted DEA registration covering Schedules II-V (Hawaii-specific DEA required).
  • Current BLS certification (AHA or equivalent).
  • CAQH profile current and re-attested per payer requirements.
  • No exclusions on OIG LEIE, SAM.gov, or Medicare/Medicaid debarment lists.
Experience

Minimum 1 year of post-licensure psychiatric experience preferred; new graduates considered with strong clinical references and demonstrated psychiatric rotation experience.

  • Clinical competence across mood, anxiety, psychotic, trauma, ADHD, and substance use disorders.
  • Comfort with telepsychiatry workflows, video documentation, and asynchronous patient messaging.
  • Sound judgment in controlled-substance prescribing, including stimulants and benzodiazepines.
  • Accurate, timely documentation that supports medical necessity and CPT/E&M coding.
  • Professional, calm communication with patients in distress and with multidisciplinary teams.
Preferred Qualifications
  • Experience in high-volume outpatient or telepsychiatry settings.
  • Experience with Practice Fusion, Tebra, or comparable cloud-based EHRs.
  • Bilingual (Hawaiian, Ilocano, Tagalog, Japanese, or other Hawaii-relevant languages).
  • Child/adolescent psychiatric experience.
Physical & Environmental Requirements (ADA)
  • Prolonged sitting at a workstation, 6-10 hours per shift during telepsychiatry blocks.
  • Standing, walking, and direct patient contact during in-person clinical blocks (4-8 hours per shift).
  • Sustained visual and auditory attention for video-based clinical encounters.
  • Manual dexterity for examinations, injection administration (e.g., long-acting injectable antipsychotics), and use of standard clinical equipment during in-person care.
  • Frequent use of keyboard, mouse, headset, and video monitor.
  • Occasional travel to Ohana offices, in-person clinical sites, and off-site events across the Hawaiian Islands.
  • Use of required PPE (gloves, mask, eye protection) per OSHA 29 CFR 1910.1030 during in-person clinical care.
  • Ability to communicate clearly in spoken and written English.

Hybrid environment. Telepsychiatry blocks occur in a private, HIPAA-compliant home office or Ohana-approved remote location with secured high-speed internet, professional video background, and freedom from interruption during scheduled clinical hours. In-person blocks occur in standard outpatient clinical settings with direct patient contact and occasional exposure to bodily fluids and infectious agents; PPE provided. OSHA, bloodborne-pathogen, and infection-control training is mandatory and provided annually.

OSHA Bloodborne Pathogen Exposure Determination: Category II (occasional occupational exposure to blood or other potentially infectious materials may occur during in-person clinical encounters and crisis evaluations).

Confidentiality & Compliance

This position requires strict compliance with HIPAA, 42 CFR Part 2 where applicable, and all Ohana policies governing protected health information (PHI), confidentiality, and information security. Violation of confidentiality obligations is grounds for immediate termination.

Compensation Range: Base pay $150,000/year plus production bonus. Final offer based on experience, credentials, and internal equity.
