Job summary

The Lakes Home Care, Inc. is seeking a licensed Nurse Practitioner (APRN) to support our home health operations in Boca Raton, FL.

The NP will ensure Medicare guideline compliance, assist with Plan of Care documentation, and conduct Face-to-Face patient encounters. Responsibilities include reviewing POC and recertifications, performing Face-to-Face encounters, supporting clinical documentation compliance, collaborating with RNs and MDs, and aiding in reducing hospital readmissions through

Qualifications

• Active Florida APRN license is required.
• Active NPI number is required.
• Medicare enrollment (PECOS) preferred.
• Experience in Home Health or geriatrics preferred.
• Knowledge of Medicare guidelines and documentation.
